Population Health Management programs to stay healthy

We know that health is more than just the great clinical care you receive. Knowing that, we offer different services to help our members. These services can help members to better manage their health and live healthier lives. Best of all, these resources are free!

Referral Process

For more information on how to use any of the programs listed below just call our Member Services at 1-866-827-2710, TTY 711 and ask to be transferred to a case manager. You can also email us at aetnabetterhealthMDCM@aetna.com. Members may disenroll at any time from programs.

Care Management

Some members have special health care needs and medical conditions. The Aetna Better Health Care Management team includes nurses who work with many health care providers, agencies and organizations to get the services and the care that you need. Learn more about our Care Management program.

Chronic Condition Management

If you have a chronic health condition, our Chronic Condition Management Program can help you stay healthy. We will mail you educational materials to help you manage your condition. In addition to printed materials, you may receive a call from one of our care management staff members. Activities and services are structured to support condition management and the highest degree of self-management. Additional information can be found here.

Healthy Kids and Healthy Adults

Promotes recommended health screenings and wellness programs for improved health outcomes. Health Education is also provided through community events throughout the state of Maryland. Members can also access additional wellness programs by registering for My Active Health – call our Member Services at 1-866-827-2710, TTY 711 and ask to be transferred to a case manager. Healthy Pregnancies, Healthy Babies

Promotes prenatal and postpartum care focusing on screenings, evidence-based interventions and collaboration with providers. Learn more about our Promise Program and Women’s health resources.

Flu Vaccination Program

Educational activities to promote annual flu vaccination. Speak to your primary care physician or visit your local participating pharmacy to obtain your flu vaccination. Learn more here.

Diabetes Management Program – Living with Diabetes

Supports members in understanding and managing their chronic condition. Learn more about additional resources here.

Pharmacy Restriction/Access

Restricts access to specific medications and assists with adjustment to pharmacy and benefit utilization as appropriate to healthcare needs.

Appropriate Use of Acute Care Settings

If you or your child has had an acute hospital stay you may receive a call from our Care Management Department. An Aetna Better Health of Maryland staff member will ask you questions related to your hospital stay and ask if:

  • You have a scheduled follow up appoint with your PCP or specialist
  • You were able to obtain your prescribed medication
  • You understand your discharge instructions
  • You have any additional needs and refer you to one of our Care Managers if needed
