Making the most of your provider’s visit

It’s important to have a provider that you see on a regular basis. This provider is called your primary care physician (PCP). Your PCP may be a provider, Physician’s Assistant or Nurse Practitioner. Your PCP may send you to other providers for care.

It will help you get the best care if you prepare for each visit. Before you go, think about what the provider needs to know about you. Be ready to ask good questions and take notes each time you visit a provider. This will help you get the most out of your time with your provider.

Remember, the more information you can give your provider, the better it will be for both of you. You should tell your provider as much as you can about your health and the way you live. This way, together, you can find out what’s wrong. 

You can also talk to your Service Coordinator about your health care needs. He or she is there to help you schedule an appointment with your PCP if you need it.
