The Aetna Better Health of California Diabetes Prevention Program is a new lifestyle change program that may help you adopt healthy habits, lose weight and reduce your risk of developing type 2 diabetes. It is available at no cost to you.
The program focuses on eating healthier, increasing your physical activity and managing the challenges associated with lifestyle changes.
Members will work with a personal coach and small group support to focus on healthy food choices and more activity. Members may qualify for tools such as wireless scales or activity trackers.

Health Homes Program
The Health Homes Program (HHP) provides services for members with certain chronic health conditions. These services help coordinate physical health services, behavioral health services and community-based long-term services and supports (LTSS) for members with chronic conditions.
You may be contacted if you qualify for the program. You can also call Aetna Better Health of California or talk to your doctor or clinic staff to find out if you can receive HHP services.
Covered HHP services
The HHP will give you a care coordinator and a care team. They will work with you and your health care providers, such as your doctors, specialists, pharmacists, case managers and others, to coordinate your care. Aetna Better Health of California provides HHP services, which include:

Early and Periodic Screening, Diagnostic and Treatment (EPSDT) services is a Medi-Cal benefit that provides a wide range of preventive, diagnostic, and treatment services, including case management, for individuals under the age of 21. Part of that benefit may also include Private Duty Nursing (PDN). PDN services are nursing services provided in a member’s home by a registered nurse (RN) or licensed vocational nurse (LVN) for a member who requires more individual and ongoing care than what would be available from a visiting nurse. This benefit may be accessed through our Care Management program.

Sometimes, you need extra care to feel healthy. If you live in San Diego County, then you may be eligible for our long-term care program. Get care where and when you need it. Our care managers will be there to help you every step of the way on your path to better health.
